A Dutch drug and mental health institute has issued a “Red Alert” over ecstasy pills molded in the shape of US President Donald Trump’s head, warning that the tablets could have potentially fatal effects.

The Trimbos Institute said Friday, August 28, that several of the pills had recently been submitted to drug-testing centers across the Netherlands. The country operates a network of facilities where people can have drugs tested to determine what substances they contain.

The institute’s warning was unusually direct: “Absolutely do not use these pills.”

Testing reportedly found that the tablets contained a high concentration of para-methoxymethamphetamine, or PMMA, a drug chemically related to MDMA, the substance commonly associated with ecstasy.

The warning is particularly concerning because PMMA can take longer to produce noticeable effects than MDMA. That delay can create a dangerous temptation to take more of the drug before the first dose has taken effect.

Why are Trump-shaped ecstasy pills dangerous?

The concern is not the shape of the tablets but what they contain.

According to the Trimbos Institute, the pills tested by Dutch drug-checking services contained a high concentration of PMMA. The substance has effects similar to those of MDMA but can carry serious risks, particularly when consumed in high doses.

PMMA has been associated with dangerous increases in body temperature and other severe toxic effects. In extreme cases, overheating can become life-threatening.

The Trimbos warning said the Trump-shaped pills could cause overheating with potentially deadly consequences.

Why delayed effects make PMMA especially risky

One of the key dangers is timing.

People taking MDMA may expect noticeable effects within a certain period. PMMA can take longer to produce effects, meaning a person may incorrectly conclude that the first pill was weak or ineffective.

That can lead to a dangerous sequence:

1. A person takes a pill containing PMMA.
2. The expected effects do not appear quickly.
3. The person assumes the dose was insufficient.
4. They take another pill.
5. The effects of both doses can then emerge, potentially causing severe toxicity.

What do the Trump ecstasy pills look like?

On the reverse side, the pills carry the markings “NL” and “Trump,” separated by a vertical line.

The distinctive design is likely intended to make the tablets visually recognizable and attractive within the illicit drug market. But the appearance of a pill cannot reliably tell users what is inside it.

That is one reason Dutch drug-testing services have become an important part of the country’s harm-reduction system.

Has anyone died from the Trump-shaped pills?

So far, there have been no confirmed reports of serious illness or deaths specifically linked to the Trump-shaped tablets, according to Trimbos spokesperson Anniek Groothuis.

Groothuis told The Associated Press that the institute was not aware of victims in whose blood PMMA had been identified in connection with the pills.

That does not mean the tablets are safe.

The “Red Alert” is based on laboratory testing of the pills and the known dangers associated with high concentrations of PMMA. The absence of confirmed victims simply means that authorities had not identified a documented serious poisoning or death associated with these specific tablets at the time of the warning.

What is PMMA?

PMMA stands for para-methoxymethamphetamine. It is a synthetic psychoactive substance that is chemically related to MDMA.

Both drugs can produce stimulant and hallucinogenic effects, but they are not interchangeable.

PMMA has become particularly notorious because of its association with severe poisoning and deaths. Its effects can also take longer to become apparent, increasing the possibility that a user will consume more before realizing the first dose has taken effect.

The risks can be especially difficult for users to judge because illicit tablets are not manufactured under controlled conditions. Two pills that look identical can contain substantially different substances or concentrations.

A pill’s logo does not guarantee its contents

The Trump design is another example of why the appearance of an ecstasy tablet provides little information about its chemical composition.

Illicit drug manufacturers frequently use recognizable logos, colors, shapes and names to distinguish different batches or appeal to consumers. Those markings are not a reliable indication of the dose or ingredients.

A tablet sold as “ecstasy” can contain MDMA, PMMA or other substances, sometimes in unpredictable concentrations.

That uncertainty is one reason Dutch authorities encourage drug checking rather than relying on the appearance or branding of a pill.

Why did the Netherlands issue a “Red Alert”?

The Trimbos Institute operates the Drugs Information and Monitoring System (DIMS), a nationwide network that analyzes drugs submitted by users.

When testing identifies a particularly dangerous substance or combination, the system can issue warnings to health professionals, nightlife organizations and the public.

The latest alert reflects concern about the combination of:

• High PMMA concentration
• Potentially delayed effects
• Risk of users taking additional doses
• Potentially dangerous overheating
• The pills being marketed as ecstasy

The warning therefore serves a broader purpose than simply identifying a particular pill design. It alerts people that a product circulating in the illicit market may contain a substance substantially more dangerous than users expect.

Ecstasy use remains widespread in the Netherlands

The Netherlands has some of Europe’s most developed drug-testing and harm-reduction systems, but recreational drug use remains illegal.

Ecstasy is particularly associated with Dutch nightlife, clubs and large music festivals.

A Dutch government report published in 2024 estimated that approximately 550,000 people had used ecstasy during the year, underscoring the scale of the market despite the drug’s illegal status.

Drug-checking services are intended to reduce harm in that environment by identifying dangerous substances before they are consumed.

Trump ecstasy pills are not new

The unusual tablets have appeared before.

In 2017, German police seized thousands of orange ecstasy pills molded in the shape of Donald Trump’s head.

The earlier discovery attracted international attention because of the highly recognizable design. The latest Dutch alert shows that Trump-themed tablets have continued to appear in Europe’s illicit drug market years later.

The recurring use of the design also illustrates a broader feature of the illegal ecstasy market: manufacturers often use recognizable public divs, brands and symbols to create distinctive tablet designs.

But the branding can be deceptive. A recognizable tablet may have little relationship to the actual substance inside.

What should users know about the warning?

The Trimbos Institute’s message is unequivocal: do not take the Trump-shaped tablets identified in its alert.

The warning is specifically tied to pills that testing has found to contain high concentrations of PMMA.

More broadly, the incident demonstrates why identifying an illicit pill by its color, logo or shape is unreliable. Chemical testing is the only way to establish what a particular sample contains.

For people who have already taken a suspected PMMA-containing substance and begin experiencing severe symptoms, the situation should be treated as a medical emergency rather than waiting for the drug’s effects to pass.

Symptoms of serious stimulant or toxic drug poisoning can include dangerously high body temperature, confusion, agitation, seizures, loss of consciousness and other severe reactions.

What does the warning tell us about the illicit drug market?

The incident is a reminder that the biggest danger of an illicit pill may not be what the buyer thinks they purchased.

A person may believe they are taking ecstasy containing MDMA, while laboratory testing can reveal another psychoactive compound altogether. Even when the intended substance is present, its concentration may be unpredictable.

The Trump-shaped pills demonstrate that branding and chemistry can tell very different stories.

The tablets may be visually distinctive, but the Trimbos testing found a chemical composition that authorities consider sufficiently dangerous to warrant a nationwide public warning.

At present, there are no confirmed reports of deaths or serious illness linked specifically to these tablets. Nevertheless, the combination of high PMMA concentrations and delayed effects is serious enough that Dutch authorities are urging people not to use them.

The message from Trimbos is therefore less about the unusual Trump design and more about what may be hidden behind it: a potentially lethal substance in a tablet that could easily be mistaken for ordinary ecstasy.
