A medical first in xenotransplantation

In a breakthrough that could redefine the future of organ transplants, a 71-year-old man in China survived more than 170 days after receiving a genetically engineered pig liver, marking the longest documented survival period for any human recipient of a pig liver implant.

The operation, led by Dr. Beicheng Sun and his team at Anhui Medical University, represents a landmark achievement in the field of xenotransplantation, the transplantation of organs between species. Doctors say the experiment demonstrates both the promise and the remaining challenges of using animal organs to save human lives.

How the transplant worked

The patient, who suffered from end-stage liver cirrhosis and hepatitis B-related complications, had exhausted all available treatment options. As a last resort, he received a genetically modified pig liver, which was engineered to minimize immune rejection and perform essential liver functions.

According to the medical team, the pig liver began functioning almost immediately, producing bile and performing key metabolic and detoxifying roles within the patient’s body. However, on day 38, the transplanted organ was removed after signs of xenotransplantation-related thrombotic microangiopathy (xTMA), a rare and serious immune reaction, were detected.

Despite the removal, the patient lived an additional 171 days with medical support, setting a new benchmark for survival in pig-to-human organ trials.

What doctors are saying

Dr. Beicheng Sun, who led the research team, described the results as a transformative moment for transplant science.

“Everyone always says the liver is too complicated to transplant compared to the heart or kidney,” Dr. Sun told CNN. “But after this, people will think differently.”

His optimism was echoed by Dr. Heiner Wedemeyer, a hepatology professor at Hannover Medical School in Germany, who called the achievement “groundbreaking.”

“It’s a cause for cautious optimism and a reminder of how far the field must still travel,” Dr. Wedemeyer said. “For me, as a transplant hepatologist, it offers completely new perspectives.”

Global progress in xenotransplantation

The Chinese case follows a series of pioneering xenotransplants in recent years:

• 2022: David Bennett, 57, became the first person to receive a genetically modified pig heart at the University of Maryland Medical Center. He lived for two months following the procedure.
• 2024: Richard Slayman, 62, underwent the world’s first pig kidney transplant at Massachusetts General Hospital. He survived for over two months before passing away.

These early trials, while limited in duration, have provided vital data on how genetically engineered animal organs behave inside the human body.

What this means for the future

Organ shortages remain one of the most pressing issues in global healthcare. According to the World Health Organization (WHO), hundreds of thousands of patients die each year waiting for transplants that never come.

If xenotransplantation becomes safe and reliable, it could revolutionize the field, offering an unlimited supply of compatible organs and saving countless lives.

However, experts stress that widespread clinical use remains years away. Complications like immune rejection, blood clotting disorders, and ethical concerns about cross-species transplants still need to be addressed through ongoing research.

“This case gives us hope, but also reminds us that nature’s boundaries must be approached with care,” said Dr. Wedemeyer.
