Nearly every heart attack, stroke, and major cardiovascular event can be traced back to the same four health issues—conditions that are common, measurable, and, crucially, treatable.

That’s the central finding of one of the largest cardiovascular studies ever conducted, which analyzed health data from more than 9 million adults in the United States and South Korea. Published in 2025, the research concludes that 99% of serious cardiovascular events are linked to four cardiovascular risk factors: high blood pressure, high cholesterol, high blood sugar, and tobacco use.

For doctors, policymakers, and patients alike, the message is hard to ignore: heart disease is rarely random—and prevention starts earlier than most people think.
